This program purchases new canine mobility carts for Corgis and Corgi mixes for whom other alternative sources of carts and funding have been exhausted. If approved, the foundation will purchase a new cart of up to $400 plus standard/ground shipping fit to your dog, to be loaned to your dog for as long as it then need it.
By filling out and submitting the form below you:
- Certify that to the best of your knowledge the dog for whom this cart is being requested is a Pembroke Welsh Corgi, a Cardigan Welsh Corgi, or is a mix including at least 25% of either of those breeds. (This program exists specifically for those breeds.)
- Certify that you are unable to purchase a cart outright, and have reasonably exhausted all alternatives including, but not limited to, the CorgiAid cart loaner program, and that without this aid it is likely that the dog will not get a cart at all in the reasonable future.
- Agree that a cart procured through the program remains the property of The KLeo Foundation, will be used only by the dog indicated, and when it is no longer needed will be returned to the foundation or to a third party designated by the foundation at my own expense.
- Agree that you will exercise due care in the use and maintenance of the cart, keeping it in good repair and in a condition equivalent in all respects to that in which it was received, normal wear and tear excepted.
- Agree to work with the cart manufacturer in a timely and accurate fashion to make sure that the cart is properly adjusted and fitted to the dog, and that the cart is used properly at all times. You understand that the cart is not to be used except under my supervision or that of another responsible party; that is, the dog should not be left in the cart without supervision. You also understand that the dog should not be off-leash in the cart except in a safe off-leash area or in a fenced yard or house.
- Certify that you will hold The KLeo Foundation and its representatives harmless from all claims, actions, proceedings, damages, and liabilities, including attorney’s fees, arising from or connected with the possession and use of the cart.
Understanding that, please provide in the contact form below all of the following information:
- Your name
- The dog’s name
- Your email address (please double check this – if we can’t contact you we can’t help).
- Your mailing address
- A brief description of your dogs situation and efforts and/or remedies attempted.
